Promotional Period:

1. The sweepstakes begins on February 28, 2011 and ends at close of survey on or before March 5, 2011 .

2. Drawing date will be held on or before March 10, 2011.

3. The prize-claiming deadline is two weeks from prize notification to the winner.

Geographic Territory:

Sweepstakes is offered within the fifty (50) United States and is open to their legal U.S. residents who are 18 years or older at time of entry, and internationally, where legal.. Void where prohibited by law.

To Enter:

Only one entry per person, per household.Your name will automatically be entered into the sweepstakes. All entries must be received by 12:00 Midnight (PT) on or before Marh 8, 2011.  to be eligible. You may also fill out the survey in its entirety, and fax it to (206) 337-0622 , or mail it in to e-FocusGroups 6189 San Bruno Court, Rohnert Park, CA 94928. All mail-in entries must be postmarked by March 5, 2011and received no later than March 8, 2011. All entries become the property of the Sponsor and will not be returned.

Cash Prize Drawings Details:

A random drawing will be held on or beforeMarch 10, 2011from all eligible entries (email, fax or mail-in). The drawing will be conducted by the Sponsor whose decision is final in all matters relating to the Sweepstakes. Cash prize will be awarded. Only one prize per person, per household.

Estimated Odds:

Actual odds of winning the prize depends upon the total number of actual eligible entries received.

Prize Description:

Five (5) prizes of $100.00 in Amazon.com gift certificates will be awarded. Approx. Retail Value (ARV) $100.00. Total ARV of this drawing is $500.00. All taxes and liability relating to the prize are the sole responsibility of the winner. Only one winner per household. No prize substitutions or transfers of prizes permitted except at the sole discretion of the Sponsor.

Notification of Winner:

The winner will be notified by phone and/or e-mail with delivery confirmation and may be required to complete and sign an Affidavit of Eligibility and Liability Release, furnishing winners' social security number which will be used only for the purpose of reporting the winner's prize to the Internal Revenue Service as required by law, which must be turned in 14-days from prize notification date or the prize will be forfeited and awarded to an alternate winner.

General Conditions:

By participating in the Sweepstakes, entrants agree to release liability of the Sponsor from any submissions, entries, claims or notices, which are lost, misdirected, damaged, incomplete, incorrect, postage due or Internet server failure. Releasee is not responsible for printing or typographical errors in the Sweepstakes related materials.

For names of winners, mail a self-addressed, stamped envelope to e-FocusGroups, 6189 San Bruno Court, Rohnert Park, CA 94928. Requests must be received no later than March 15, 2011.
